All client forms for new or existing clients should be completed in your TherapyPortal.

Please note that all intake forms for new clients must be completed 24 hours in advance of your first appointment or your appointment is subject to cancellation.

To authorize us to release or obtain your healthcare information, please email: admin@aoclinic.org and a consent form will be placed in your TherapyPortal for completion.

Third Party Payors:

If you are a third party who agrees to pay for a client’s sessions, please contact our office at 301-767-1733 ext 0 or info@aoclinic.org to request a Third Party Payor Agreement.
